Hmm I can't quite remember. It depends if the store has an application form! Usually for part time jobs (I assume you're asking with relation to yogurt/icecream shops), they don't really need your resume haha. But if you really must, I think personal details (name, contact number, email etc) and previous jobs may help!

I emailed them and then filled in a form, got called for an interview so I went down. Yeah I attached a picture but it wasn't the passport kind, I just used a random fb photo haha

I wanted to volunteer with AWWA or Rainbow Centre, but I couldn't because they need volunteers to stay at least for 6 months, and I didn't have the time to :(

Im not quite sure.. don't think it's alot unless its for the church's resources? Expenditure for the missionaries are minimal.
