Most cock rings are designed to be worn around the base of the cock and balls, though some vibrating cock rings work nicely around just the shaft.

If it's made of a stretchable material, you stretch it out with your fingers, like a rubber band, slide it over your cock and balls, and then slowly remove your fingers to allow the band to constrict. Some rings are made of leather with a snap closure. Cocks rings made of solid material (an O-ring of metal, jade, etc.) just slide over your cock and balls and fit snugly at the base.
