I made a decision a few years ago to concentrate all my charitable giving into one unimpeachable organization, Doctors Without Borders. It saves me from having to research alternatives, and in any cases the chances of any alternative being significantly better are slim indeed. Now, I give nowhere near $1 million to DWB, but that's not enough money for me to significantly change my policy, unless I wanted to do something a little bit more selfish like support my friends' organizations locally, or their kids' schools. Or essentially procure for myself free tickets to the Metropolitan Opera whenever I wanted them. That kind of thing. I'm sure I could maximize reciprocating gratitude elsewhere. But in terms of easy and obvious bang-for-buck, I think DWB it would be.
