First off, that's one of my favorite books. Second, paradise will never happen, because utopias are dystopias in disguise.

It's definitely the choice of the woman who's carrying the child, as well as her partner if such is the case. It shouldn't be used as a method of birth control, but it should be an option for those women who simply aren't ready to have a kid yet, or can't be pregnant for whatever reason. Taking away the ability to have a procedure means you have more abortions done in sub-par circumstances, and that leads to a higher risk of the woman dying.
