That is a classic over/under setup that I love for bright beauty looks. A large softbox on top and a medium softbox underneath. Both at nearly 45 degree angles pointed towards the face, not exactly that but close to that.

Well, usually you're shooting someone from at least five feet away, and at that distance a slight change in direction they face is enough to take the photographer outside of the angle-of-reflection with very little change to the composition.
But if they absolutely must look DIRECTLY at you then the clone stamp is usually the easiest solution. -
I don't. I select the images that are delivered. In the cases where somebody really wants to choose themselves I export selected images to a web gallery using Lightroom and FTP them to a private URL for viewing.

My past world travels have been done without my camera, though I may change that in the future. Hard to say for sure.

I use grids on almost every softbox I use, however, when the softbox is really big (like the 5' octas) I noticed that the grids don't make a whole lot of difference. There are third-party companies that make grids for the Rotalux softbox though so I wouldn't avoid buying it just because of the lack of grid.

My studio has 10 foot ceilings, which works ok but I would definitely prefer higher sometimes.

I mainly use a white cyclorama built with Masonite boards.
